 Fees and Parental Consent:
Students and Parents will NOT BE CHARGED for any services students received in the clinic; services are free to any student enrolled at schools where clinics are located.

Clinic Consent Form
Students must have a clinic consent form on file at the clinic. If you do not, please stop in and pick one up. Minnesota law requires a parent or guardian signature for all students under 18 years old. Exceptions include:

• Minors living independently
• Services related to pregnancy
• Pregnancy prevention
• Sexually transmitted disease
• Alcohol or other chemical abuse
• Medical/mental health emergencies

Laboratory Services / Screening:

• Pap Smear

Wellness Promotion:

• Immunizations (Measles, Mumps, Rubella, Tetanus, Polio, Hepatitis B)
• Smoking Prevention and stop smoking class
• STI / STD Prevention Education
• Safety Promotion
• Classroom Presentations

Reproductive Health:                                                  

• Abstinence Counseling
• Contraceptive counseling
